Thank you for your interest in elder care services in Sonoma County and the surrounding area. My name is Wendy Taylor and I established Taylor Elder Care Management in September of 1998. I have a Master's degree in Social Work and have been a member of the National Association of Geriatric Care Managers since 1993.

I have an extensive background in working in Hospital, Home Care, and Rehabilitation settings with an emphasis in Immediate and Long Term Planning for elders and disabled adults.

As a professional care manager, my goal is to maximize the independence of the elderly or disabled adult client. The initial assessment by Taylor Elder Care Management will include an evaluation of the lowest cost approach to serve the client.

When care management services are needed, an individualized care plan will be developed which reflects the clients beliefs and wishes for quality of life and allows for positive change.

Oftentimes, the client and family will have existing services already in place to support their needs and goals. My aim is to maintain the client's existing services and family support and supplement them with needed services when required to increase safety and quality of life.
