I have worked as a geriatric care manager for 12 years serving the elderly and disabled adults. During this time, I have had the opportunity to meet some truly amazing people including clients, their families and their caregivers.

Throughout the years, I have created a staff of case managers and nurse consultants. We have come to recognize that, with the right assistance to the family and the client, an individual can maintain his/her independence and avoid institutionalized care.

We create a team approach, coordinating with other geriatric specialists and agencies to support the family. Our goal is to provide this team of supportive services, which will allow for the client to remain in a lower level of care with maximum independence.

With this level of support and coordination, the client is able to make many of his/ her own decisions and maintain a sense of dignity. We have a medical model and provide 24 hour on-call availabiltiy of our staff for an immediate response to problem areas. 

With necessary modifications for increased safety and with coordination of in-home care and community networking, elders can often live out their lives in their homes or in the least restrictive environment that can meet their care needs. The quality of life for an elder or disabled adult in your family is our number one concern.

We believe that, with the help of a case manager to make the connections between family, community and geriatric specialists, our client's quality of life can be the highest priority even through difficult transitions such as coping with chronic illness or adapting to the aging process.
