As a case Manager I assist with making decisions that include: 

  • Visits with the client in his or her current living environment to provide assessment of care needs.

  • Assistance with contacting a placement specialist. 

    A placement specialist provides free guidance on Assisted Living Settings, Board and Care settings and rules and regulations for Residential Care Facilities. We use Tweeten Eldercare Advisors

  • Finalizing placement and setting up a care plan.
